Dance Liablity Waiver
I understand and agree that in participating in Jornae Dance’s class that there is a possibility for physical injury or death. I voluntarily agree, therefore, to assume all risk and responsibility for any such injury or accident, which might occur to me or my child during Jornae Dance classes. I also exempt, release and indemnify Jornae Dance and its owners, assistance, and students from any and all liability claims, demands, or causes of actions whatsoever from any damage, loss, injury, death, or property which may arise out of or in connection with participation in any classes or activities conducted by Jornae Dance. I further hereby voluntarily agree to waive my rights and that of my heirs and assigns to hold Jornae Dance, its owners, assistance, and students liable for such damages, loss, injury, or death. In regards to Covid 19, JornaeLLC will not be liable for any transmitting or deaths due to the virus. I understood that I should be aware of physical limitations and agree not to exceed them. If I am signing this waiver for my children, I certify that I am the parent or legal guardian and have the right to waive these rights. Furthermore, permission is granted to Jornae Dance to use any photographs and/or videography of students for advertising and publicity purpose.
